  • Completed and Signed Account Opening Form
  • Applicants’:
    • Recent Passport Sized Photo
    • Copy of National ID / Valid Passport/ Birth Certificate (with attested photo ID)
    • Copy of TIN (Optional)
    • Income Proof Document (If required)
    • Copy of Recent Utility Bill-Gas/Electricity/WASA/Telephone
  • Nominees’: (attested by Applicant)
    • Recent Passport Sized Photo
    • Copy of National ID / Valid Passport
  • During Account Opening, Branch may request for any other appropriate document(s).
